Sample Transcripts Transcript Information Form LETTER TO SCHOOL
This is a sample letter that you can send to the public or private high school your child is to enter to find out exactly what they will want in order to count any homeschool credits. Policies vary from district to district, and even school to school within a district. If the principal changes at one school, there may be changes in the homeschool acceptance policy.
If you are doing your own transcript, use this format as a sample and customize it to your situation. If you are having your transcript prepared by Covenant Home School Resource Center/H.O.M.E. School Books and Resources, please print off the PDF version of the form and submit that with your other paperwork.

2. If you do not have a written policy, what are the items you will require of the student to accept the credits? What format is needed for these? What standards will you use in evaluating these?
If the student produces these items satisfactorily, will the credits automatically be validated?
If not, what else will be needed?

4. What is your policy on fractional credits (1/4 or ½) ?
5. What is your policy on transferring of home school grades to the school transcript?
